How claim appeal works
Most payers define multiple levels of appeal, starting with an internal reconsideration and escalating to a formal appeal or, in some cases, an external review by an independent body. Each level has its own filing deadline, commonly counted from the date of the denial, and missing that window generally forfeits the right to appeal at all.
A strong appeal includes the original claim, the denial reason, the specific plan or policy language being challenged, and supporting documentation such as medical records or a prior authorization number. Practices track appeal outcomes to see which denial reasons are worth appealing and which ones point to a process fix needed upstream, such as tighter eligibility verification.

A claim for a $1,200 procedure is denied for lack of medical necessity. The billing team files a claim appeal with supporting clinical notes and the treating physician's documentation within the payer's 60-day window. The payer reverses the denial six weeks later and pays the full $1,200, which the billing team then reconciles against the corrected claim record.

- Not escalating through the payer's defined appeal levels when the first attempt fails, which leaves an internal reconsideration as the final word instead of pursuing an external review.
