# Insurance verified, claims clean, patients billed

LedgerBPO's dental billing services give your practice a named, HIPAA-trained biller who verifies insurance before the appointment, submits clean claims with attachments from Dentrix or Open Dental, posts EOBs, works denials and follows up on patient balances under your practice name. The same team reconciles deposits to your books, so production, collections and bank agree each month.

## What is dental billing outsourcing?

Dental billing outsourcing means a remote team runs the insurance and patient billing cycle for a dental or therapy practice: verifying benefits, submitting claims with attachments, posting explanation of benefits, correcting denials, filing secondary claims and following up on patient balances. The practice keeps its own practice management software; the provider works inside it under a business associate agreement.

## Who it is for

### Dental practice owners

One to five dentists, a front desk that handles scheduling and insurance, and claims that go out when someone finds time. Unposted EOBs and unworked denials are hiding in the aging report.

### Office managers

You verify benefits, submit claims, post payments and call patients, all between check-ins. A dedicated remote biller takes the back-office half so the front desk can focus on patients.

### Behavioral health and therapy practices

Counselors, psychologists and ABA providers on TherapyNotes or SimplePractice have the same cycle with different codes and authorizations. The same team handles it.

## What's included

- Insurance breakdown verification: frequencies, waiting periods, annual maximums, remaining benefits
- Pre-treatment estimates and predeterminations for major work
- Claim submission from Dentrix, Open Dental or Eaglesoft with x-rays, narratives and perio charts
- EOB and ERA posting with write-offs by fee schedule and PPO contract
- Denied and rejected claims corrected and resubmitted within timely filing
- Secondary claims and coordination of benefits after primary payment
- Insurance AR follow-up by carrier and aging bucket
- Patient statements and balance calls under your practice name
- Credit balance and refund review each month
- Daily deposit reconciliation between the practice management system and the bank
- Behavioral health claims and authorization tracking on TherapyNotes or SimplePractice
- Monthly report on production, collections, adjustments and AR by carrier

## Deliverables and KPIs

| Deliverable | KPI we report | Cadence |
| --- | --- | --- |
| Eligibility verified | Benefits confirmed at least 2 business days before the appointment | Daily |
| Claims submitted | Claims with attachments sent within 1 business day of the visit | Daily |
| EOBs posted | Payments posted within 1 business day of receipt | Daily |
| Denials worked | Every denial touched within 5 business days with a next action | Weekly |
| Patient statements | Statement cycle run on your schedule; balances explained on request | Monthly |
| Deposit reconciliation | Practice management deposits tied to the bank and ledger with variances explained | Monthly |

KPIs are what we measure and report, agreed per engagement. Service-level commitments are set in your agreement.

## How it works

1. ### Billing audit call
A 30-minute call to map your providers, carriers, PPO contracts, practice management software, monthly claim volume and where claims currently stall. We look at your insurance aging and a sample of recent denials.
2. ### BAA, access and carrier setup
We sign a business associate agreement and you grant role-based access to your practice software, clearinghouse and carrier portals. Fee schedules, attachment rules and statement preferences are confirmed in week one.
3. ### Dedicated biller assigned
A named biller, a trained backup and a team lead take over verification, claims and posting. The first two weeks run in parallel with your current process so nothing is missed.
4. ### Daily queues, monthly reconciliation
Verifications, claims, postings and denials are worked every business day. At month-end the team reconciles deposits to the bank and your ledger and sends a short report on collections and AR by carrier.

Our DunningDesk process

Patient balances follow our DunningDesk cadence: a statement, a courteous reminder, a call with payment plan options, and a promise-to-pay log, all under your practice name. Every touch is recorded against the account, so the front desk can see the history when the patient next visits.

## How much does dental billing cost?

Dental billing is commonly priced as a percentage of collections, per claim, or per dedicated biller. Single-location practices often start with a dedicated part-time biller and scale as claim volume grows, and engagements run month-to-month. Pricing depends on volume and scope, so we send a custom quote within 1 business day.

### What moves the quote

- Monthly claim volume and number of providers
- Number of carriers and PPO fee schedules to maintain
- Whether insurance verification and patient calls are included
- Size of the insurance and patient AR backlog at the start

### Market benchmarks

| Market | Typical range | Source |
| --- | --- | --- |
| US | Dedicated offshore billing staff typically run $8 to $35 per hour all-inclusive, against $40 to $75 per hour for US-based staff | Madras, Acculink, Rose Talent (2026) |

## Frequently asked questions

### How much do dental billing services cost?

Pricing depends on volume and scope, and we send a custom quote within 1 business day. Dental billing is commonly priced as a percentage of collections, per claim, or per dedicated biller, and we recommend the model that fits your claim volume and carrier mix. As a market reference, dedicated offshore billing staff typically cost $8 to $35 per hour all-inclusive (Madras, Acculink, Rose Talent, 2026).

### Do you work inside Dentrix and Open Dental?

Yes. We work inside your existing Dentrix, Open Dental, Eaglesoft or similar practice management system and your clearinghouse. You keep the subscription, the patient records and the carrier logins. Nothing is exported into a system of ours, so your front desk and our biller always look at the same ledger.

### How does insurance verification work?

At least two business days before each appointment the biller confirms coverage, frequencies, waiting periods, annual maximum and remaining benefits with the carrier portal or by phone, and records the breakdown in your practice software. The front desk then knows the patient portion before treatment. Our insurance verification outsourcing page explains the call desk behind this.

### What attachments do you send with claims?

Whatever the carrier requires for the procedure: x-rays, intraoral photos, perio charts and a narrative for crowns, scaling and root planing, implants and other major work. Missing attachments are the most common reason dental claims are denied, so the biller checks each claim against the carrier's rules before it leaves.

### Do you handle behavioral health and therapy billing too?

Yes. Counselors, psychologists and ABA providers on TherapyNotes, SimplePractice or similar systems get the same daily cycle with the right CPT codes, session limits and authorization tracking. The workflow is close to a dental practice's, which is why one team can serve both.

### How do you follow up with patients about balances?

Patient statements and calls go out under your practice name on our DunningDesk cadence, with respectful scripts, clear explanations of insurance adjustments and payment plan options. Agents never store card data; payments run through your practice portal. All calling follows US TCPA rules and your practice's policy.

### What about the AR that has built up already?

We start with an AR sprint. Insurance claims are sorted by carrier, age and timely-filing risk and worked first; patient balances are sorted by amount and last contact. You receive a written summary of what is collectible and what should be adjusted, and you approve every write-off before it is posted.

### Can you reconcile collections to my books?

Yes. The same team that posts EOBs ties practice management deposits to your bank statement and QuickBooks Online or Xero ledger each month. Insurance payments, patient payments, merchant fees and refunds are each explained, so your financial statements match what the practice actually collected.
