Because a deposit from a payer combines many sessions and many clients, arrives net of recoupments, and says nothing about the contractual adjustment on each claim. Card-on-file payouts combine copays from a whole week and subtract processing fees. Posting from the practice-management payment report and matching it to the bank is the only way to know collections per session and per clinician.

### How are sliding-scale fees recorded?

A sliding-scale session is recorded at the reduced fee the client agreed to, with the difference from your standard fee tracked as a fee reduction rather than as bad debt or a payer adjustment. This keeps the effective fee per session honest, shows how much revenue the sliding scale gives up, and stops reduced-fee sessions from inflating the client aging.
